OmniForm Mailable Filler (Recommended method for PC users)
OmniForm Mailable Filler is an executable file that enables you to download a form to a diskette or your hard drive, fill out the form on screen, and save your work. From there, you can print a copy, fax a copy (via hard copy or FAX modem), and/or send the file to an e-mail recipient. OmniForm Mailable Filler forms will only run on PCs equipped with Windows 95, 98, NT, 2000 or XP systems. (If you experience any difficulty, you may wish to to try the Adobe Acrobat version of this form instead.)

Adobe Acrobat (Recommended method for Non-PC users)
Adobe Acrobat Portable Document Format (PDF) files can be printed and then used in the same manner as any traditional paper form. They can be viewed and printed with Adobe Acrobat software, including the free Acrobat Reader. This software is available for use with several different computer operating systems, including Windows 3.1, 95, 98, NT, 2000, XP, OS/2, Macintosh,and several Unix versions, and can be obtained by clicking on the button below.

If you have the Acrobat software configured as a web browser plug-in application you can fill-out and print the form within your web browser. If you are using the free Acrobat Reader software, however, you cannot save the form with the fill-in data for use at a later time. You will need to use the purchased version of Adobe Acrobat Approval or Adobe Acrobat in order to do this. The free reader software will, however, allow you to save the blank form itself for use at a later time if you are using the filler software as a web browser plug-in application.

To Download

  1. Click the link to the form you want to download. The File Download dialog box displays. 

  2. Select Save this program to disk, then click OK. The Save As dialog box displays. 

  3. Click the down-pointing arrow beside the Save In window. 

  4. Click the hard disk or other disk drive and the folder in which you want to save the file. Then click the Save button.


To Fill Out An OmniForm Mailable Filler

  1. On your hard disk or other disk drive, locate the file you saved.

  2. Double-click the file name. The form displays, and the OmniForm Mailable Filler dialog box displays.

  3. Click OK.

  4. To move to another page of the form, click a page icon at the bottom of the form window.

  5. Click in the first yellow space of page 3. Enter data, then press Tab to move to the next field.

  6. Check boxes are marked by using the Space Bar. To backup a field for correction, etc., use the Shift & Tab key combination.

  7. If you want to save the data you entered on the form, click File, then Save.
  8. When you have completed the form, click File, then Print to print a copy.
